Wow! Thats a question and a half, which will be a difficult question for most to even contemplate.
However i do believe that we have the right to choose our own destiny and i am a supporter of the Dignity in Dying. So i guess my answer would be that i would be supportive of my friend in every way and assist where necessary.

the idea is fundimentally a logical one. However, we are not logical beings. The ramifications of psychological damage to families involved is a huge can of worms. The doner child may feel like they were only wanted to provide life for their sibling and not truly loved and second best to the donee leading to resentment. The donee child may resent the doner child as they would be the "savior" and therefore feel indebted to them for life. The parents would be the key, treating each child equally and fairly without favourites etc. And that's just the very very very basics without any other emotions, ramifications etc. How would a child fee if as a doner, the treatment didn'twork, how would the parents/sibling treat them etc. Its a morilty, ethical, logical can of worms.
which then begs the question, just because we can do something, should we?
